-- Tejas Grocery, Hwy. 418 and Hwy. 69, Kountze, 65. Milk cooler dirty and needed cleaning. Slime on ice machine. Cook had no food training. Rotten chicken stored in cooler. Came back and everything improved to a 90.

-- Chef Chen, Hwy. 96, Silsbee, 70. Surprise visit. Roaches crawling in kitchen area. Gnats in kitchen and around fountain drink machine. Nor washing hands at hand sink. Water not hot enough for wash, rinse and sanitizing. Was told to get pest control out for roach problem. Had to go back two times until mostly everything was corrected.

-- Grand China, 132096 S. Main, Lumberton, 86. Can't have anything in hand sink but hands. No paper towels at hand sink. Need dates on food in freezer. Everything needs to be up off floor six inches in WIC. Container lids dirty and need cleaning.

-- Tigerland Quick Stop, 1175 Hwy. 418, Silsbee, 96. Out of date can goods on shelf.

-- Dairy Queen, 1290 South Pine, Kountze, 96. Improper scoop in gravy.

-- CVS Pharmacy, 100 South LHS Dr., Lumberton, 97. Needed thermometer in milk cooler.

-- Jack-N-The-Box, 150 Hwy. 327, Silsbee, 97. Opened for new business. Hand sink cannot be used for anything except washing hands.
